The following documentation is required for individuals and families seeking assistance with deposits for utilities.
1. FEMA Determination Letter

7. Copy of Documentation from Utility Company Verifying Deposit Amount
8. Two Months Bank Statements for all Accounts

Home Repair Documentation Requirements
The following documentation is required for individuals and families seeking assistance with repairs to their home.
1. Proof of property ownership
A. Warranty Deed
2. Proof that you are current in your property taxes (One (1) item needed for this category)
A. Property tax payment receipt from City
B. Cancelled check for property taxes to the City
C. Affidavit certifying payment of property taxes
D. Mortgage statement from your lender saying taxes were paid
3. Proof of Homeowner or Hazard Insurance
A. Proof of Hazard Insurance
B. Proof of Homeowners Insurance
4. Proof of number of dependants claimed.
A. Birth Certificates of children listed on application.
B. Social Security Cards
C. School Records that give the parents’ names and address. (if applicable)
D. Court ordered letters of Guardianship. (if applicable)
E. Divorce decree (if applicable)
F. Letters of adoption (if applicable)
5. Driver’s License or Florida ID Card (on any household member eighteen (18) years or older)
6. Proof of gross income from all family members over eighteen (18) years old in household
A. Last two (2) Pay Stubs (if applicable)
